Silicone oil used in this case was for retinal detachment repair and has emulsified/ It has come to the anterior chamber. The oil is less dense than aqueous fluid and will be found in the superior angle. This is sometimes referred to as a "reverse hypopyon".

The patient presented with a RAM and sub-macular hemorrhage. Treatment was with pars plana vitrectomy, sub-retinal tissue plasminogen activator injection and 26% SF6 gas. The hemorrhage resolved with some photoreceptor changes, but no fibrosis and the patient regained 20/40 vision.
