Primary Vitreoretinal Lymphoma
A 77-year-old female with a history of vitritis of unknown origin and two non-diagnostic vitrectomies presented with a white plaque on the posterior capsule of her left eye after being lost to follow up for several years. Repeat vitrectomy confirmed the suspected diagnosis of primary vitreoretinal lymphoma (PVRL). MRI showed enhancing lesions of the brain, consistent with CNS manifestation of the lymphoma as well. PVRL is commonly associated with Primary CNS Lymphoma, and both are most often of B-cell origin. PVRL can be a diagnostic challenge, as it is a “masquerade syndrome,” which is responsive to corticosteroids just like noninfectious uveitis. As in this case, multiple biopsies may be necessary to obtain a definitive pathological diagnosis.
