Acquired Corneal Subepithelial Hypertrophy
A patient presented to the Bascom Palmer Eye Institute with decreasing vision in her right eye several months after having a penetrating keratoplasty for ruptured radial keratotomy incisions. Her exam was notable for visual acuity of 20/60 in the right eye and 20/25 in the left eye. Slit lamp exam was most notable for a temporal area of a white, crescent-shaped, elevated corneal opacification. Corneal topography showed significant irregular astigmatism induced by the corneal opacity. The patient underwent superficial keratectomy, phototherapeutic keratectomy, Mitomycin C 0.2%, and amniotic membrane graft. There was improvement in the visual acuity of the right eye from 20/60 to 20/30 at post-operative month 1 and post-operative corneal topography showed improvement in the irregular astigmatism.
