Age-Related Macular Degeneration
We present an elderly patient with new onset visual distortions in the right eye. Vision is 20/30 OD, 20/20 OS. She is diagnosed with exudative Age-related Macular Degeneration (AMD) in the right eye and non-exudative AMD in the left eye. Swept Source OCT Angiography (SS-OCTA) helps us identify evolving Retinal Pigment Epithelial (RPE) tear in the right and subclinical Macular Neovascularization (MNV) in the left. Anti-VEGF therapy has been implicated by some as a cause of RPE tears due to progressive contraction of the MNV. The question arises whether to treat the right eye with anti-VEGF injections in the setting of evolving RPE tear. Given that there were no signs of exudation in the left eye, the patient is observed closely.
