Grand Rounds
An adult patient presented to Jackson Memorial Hospital with complaint of blurred vision, fever, abdominal pain, weight loss, and rash. Examination revealed a chorioretinal lesion concerning for toxoplasma chorioretinitis. The patient was initiated on broad spectrum IV antibiotics. Laboratory work up and anterior chamber sample PCR revealed a diagnosis of HIV/AIDS as well as concurrent intraocular CMV and toxoplasma infections. The patient received additional intravitreal injections of foscarnet, ganciclovir, and clindamycin. He tolerated the treatment well and experienced subsequent improvement of the blurry vision and vitritis. However, follow-up was complicated by new blurred vision and recurrence of toxoplasma chorioretinitis due to a mix of poor medication adherence and immune recovery inflammatory syndrome.
