Granular Corneal Dystrophy Type II

A patient presented to the Bascom Palmer Cornea Clinic with progressively blurry vision in both eyes as well as worsening pain and irritation in both eyes. She had a family history of corneal problems. Her visual acuity was 20/100 in the right eye and 5/200 in the left eye. Intraocular pressure was within normal limits and pupils were equally round and reactive. Anterior stromal corneal opacities were noted in both eyes. Anterior segment OCT also showed anterior stromal corneal opacities in both eyes. The presumptive diagnosis of granular corneal dystrophy type II was made. Different treatment options were discussed with the patient, and the patient preferred to avoid corneal transplantation if possible. Cataract surgery was performed first in the left eye. Phototherapeutic keratectomy was then performed in the left eye followed by the right eye. Patient's visual acuity improved significantly, and patient's pain and irritation improved after treatment. The patient is currently doing well s/p PTK on artificial tears and autologous serum tears.
