A 71-year-old male with a history of atopic dermatitis was referred to BPEI for evaluation of a corneal lesion. Exam revealed an opalescent and gelatinous lesion at 2-4 o'clock at the limbus in the left eye. Anterior segment OCT showed a thickened, hyperreflective epithelium with an abrupt transition. Biopsy confirmed ocular surface squamous neoplasia. He was treated with a four-week course of 5-fluorouracil, and complete resolution of the lesion was obtained. However, two years later he presented with recurrence. He was again treated with 5-fluorouracil, however the lesion did not completely regress. He was then treated with surgical excision and cryotherapy and is now in complete remission. His treatment course is documented in this presentation.
