Abstract
A 68-year-old male with restrictive strabismus and Brown syndrome with extraocular movement restriction secondary to TED. He was on clinical trial for Teprotumumab 302 and had prior RAI ablation. His visual acuity OD is 20/20, OS 20/20. He had limitation elevation of the left eye in adduction and intorsion most consistent with Brown syndrome secondary to tight SO. His intraoperative forced ductions revealed severe restriction to elevation in adduction and mild to moderate restriction of elevation, abduction and depression in left eye. He underwent bilateral medial rectus recessions with SO expander in left eye. His postoperative course showed stable alignment.
This presentation discusses the mechanism, diagnostic evaluation, management and outcomes in a case with restrictive strabismus and secondary Brown syndrome due to TED that was successfully treated.
