Abstract
A young patient with a past ocular history of Stevens Johnson Syndrome and prior Candida and Providencia keratitis presented with multifocal white corneal infiltrates several months following a penetrating keratoplasty of the same eye. The patient’s exam was notable for a vision of 20/200 and intraocular pressure of 22 in the left eye. Slit lamp examination demonstrated 2 central white infiltrates with 2 adjacent satellite infiltrates within the penetrating keratoplasty which spanned the anterior stroma. Corneal cultures demonstrated Candida albicans which was initially treated medically with topical voriconazole and cessation of topical steroids. Treatment was escalated to include concurrent topical Amphotericin B and intrastromal Amphotericin B injections when the infiltrates remained refractory to initial therapy. The fungal infiltrates consolidated into a central scar with moderate corneal thinning with negative fungal cultures subsequently. The patient’s vision returned to baseline.
