Pseudophakic Bullous Keratopathy, Corneal Edema
A patient with a history of penetrating keratoplasty of both eyes, Fuchs endothelial cell dystrophy in both eyes and herpes simplex keratitis of the left eye presented with decreased vision in the right eye over the past several months. Slit lamp exam was notable for subtle corneal edema of the right eye. The patient sought non-surgical interventions to improve her vision and to avoid surgery. Given the decrease in vision in the right eye, topical steroid use was initiated with no improvement. Several months after the initial visit, no improvement was noted. Off-label use of netarsudil was discussed with the patient and the patient agreed to proceed. After using netarsudil for several weeks, the patient achieved an uncorrected distance visual acuity of 20/30 with resolution of corneal edema. The patient tolerated the treatment well, and this case report further adds to the body of literature that supports the use of rho kinase inhibitors for the management of corneal edema in transplanted corneal grafts.
