Abstract
Our patient with a history of HTLV-1 related lymphoma who was in remission for several years presented with decreased vision in the right eye for about 1 week. Initial visual acuity was 20/50 in the right eye and 20/25 in the left eye. Posterior segment exam revealed signs of severe vasculitis and retinitis in the right eye while the left eye was normal. Given her significant medical history, systemic workup was performed revealing active systemic HTLV-1 related adult T-cell lymphoma that was present systemically, including in the central nervous systemic. A diagnostic vitrectomy was eventually performed which confirmed the presence of neoplastic T-cells in the vitreous consistent with ATL-related vitreoretinal infiltration. The patient was treated systemically with chemotherapy and locally with intraocular chemotherapy. Unfortunately, the patient’s condition progressed even with aggressive treatment, leading to systemic decompensation and death.
