Pseudophakic Bullous Keratopathy with Prior Descemet Membrane Tears due to Forceps Trauma
A patient with a history of forceps trauma at birth and amblyopia of the left eye who presents with persistent corneal edema 1 year after uncomplicated cataract surgery. Initial visual acuity was 20/200, intraocular pressure was 10, and pachymetry was 698 um. On clinical examination there was noted to be apparent breaks in Descemet’s membrane consistent with prior forceps trauma at birth as well as significant edema without a very visually significant amount of corneal haze/ scarring. The patient underwent uncomplicated Descemet Membrane Endothelial Keratoplasty (DMEK). Pathology showed fibrotic bands on the endothelium consistent with a diagnosis of prior forceps trauma without guttae or other notable endothelial pathology. This patient did not require any re-bubbling or develp any complications in the recovery period. At the patient’s most recent post operative appointment (post operative month 3), vision improved to 20/70-2 and the cornea was clear.
