Abstract
A young child with no past ocular history presents with vision loss for 2 months. Her exam was notable for visual acuity of 20/250 in the affected eye with intraocular pressures of 38, relative afferent pupillary defect. Slit lamp findings were notable for subtle bupthalmos and 0.95 cupping with pallor in the right eye. The patient was started on oral acetazolamide as she is unable to tolerate eye drops. An exam under anesthesia was performed which showed enlarged corneal diameters of 13mm in the right eye and 12mm in the left eye. UBM and gonioscopy showed open angles. B-scan showed moderate cupping in the right eye and mild cupping in the left eye. A-scan showed enlarged axial length larger in the right eye than the left eye. The patient underwent gonioscopy assisted transluminal trabeculotomy of the right eye. The patient is now doing well with controlled intraocular pressure in the right eye.
