Exudative Age-Related Macular Degeneration

A patient with neovascular age-related macular degeneration in both eyes has been receiving intravitreal aflibercept in her left eye every 4-5 weeks, for more than 10 years. Whenever the interval between the intravitreal injections was increased there was recurrence for subretinal fluid and or increase in the size of the pigment epithelial detachment (PED). At her current visit the Optical coherence tomography showed an increase in subretinal fluid with a PED in the left eye and an RPE tear causing hyper transmission defect and subretinal scarring in the right eye. She was switched to faricimab in the left eye which resulted in the resolution of the subretinal fluid. The interval between intravitreal faricimab was gradually increased. Now we have been able to extend the treatment interval to 7 weeks with maintenance of stable visual acuity and a dry macula.
