Neovascular Glaucoma
A patient with history of type 2 diabetes presented with pain and acute decrease in the vision. Vision was poor and intraocular pressure (IOP) was markedly elevated. Exam revealed evidence of diabetic retinopathy with neovascularization of the iris and the angle. Neovascular Glaucoma (NVG) represents a severe sight-threatening condition, characterized with the formation of anterior segment neovascularization with resulting progressive angle closure and IOP elevations. Treatment focuses on both IOP control and suppression of the neovascular drive with anti-VEGF agents and retinal photocoagulation; prognosis is highly variable. Traditional classification of NVG may in some cases fail to accurately describe the disease state. A novel classification system, with attention to angle status, was recently proposed.
