Granular Cell Tumor

A patient presented to the Bascom Palmer emergency room with complaints of diplopia in right gaze, frontal headaches, and dizziness. Exam demonstrated proptosis of the left globe with mild resistance to retropulsion and a left adduction deficit. MRI of the orbit identified an intraconal, well circumscribed lesion adjacent to the left lateral rectus extending to the orbital apex. Orbitotomy with biopsy of the lesion demonstrated morphologically bland cells with granular cytoplasm which stained positively for CD68, SOX10, S100, and NSE. The diagnosis of granular cell tumor was made. The lesion was not easily resectable, so the patient was referred to radiation oncology and genetic sequencing was performed to assess for alternative treatment options. Ultimately, the decision was made to observe the patient with serial imaging.
