Potts Puffy Tumor
A patient with no relevant past ocular history presented with a 1-day history of intractable headache and periorbital edema. Examination showed periorbital edema with erythema of the right upper lid with associated chemosis. MRI of the brain and orbits revealed frontal sinusitis, right frontal sinus mucocele with associated osteomyelitis and pachymeningeal enhancement, as well as focal soft tissue swelling of the forehead most consistent with Potts Puffy Tumor. The patient was urgently transferred to the hospital where she was started on intravenous antibiotics. She was taken to the operating room with the neurosurgical team, where a bidirectional approach from both the orbit and superiorly was used to drain the mucocele and wash out the sinus. Over the course of the following months, the patient made a full recovery with no residual neurologic or ophthalmologic complications.
