Primary Vitreoretinal Lymphoma

A patient with no significant past ocular history presented with floaters and decreased vision in the right eye for a few days. The exam was notable for trace vitritis and multiple small yellowish deep retinal lesions in the posterior pole extending to mid periphery in the right eye. Left eye exam was unremarkable. The patient was treated with a short course of oral steroids and the lesions resolved. 13 months later, the patient had similar lesions appearing in the left eye that then spontaneously resolved without treatment. 32 months after initial presentation, the patient had new onset of significant vitritis in the right eye. Diagnostic vitrectomy was performed and the patient was diagnostic with bilateral primary vitreoretinal lymphoma. The patient was treated with systemic high dose methotrexate and bilateral globe radiation and is currently being monitored without disease recurrence.
