Giant Cell Arteritis

A patient presents with vision loss of the right eye associated with headache, jaw fatigue. A relative afferent pupillary defect was noted in the right eye. Exam was notable for optic nerve edema of the right eye and laboratory evaluation showed elevated ESR and CRP. Patient left against medical advice prior to brain imaging but re-presented five days later with temporary vision loss in the left eye. Exam at that time was notable for right eye optic nerve edema and pallor. Patient was transferred to the University of Miami hospital for further evaluation and treatment with IV steroids. After discharge, patient was evaluated in neuro-ophthalmology clinic and found to have decreased vision with cilioretinal artery occlusion. Temporal artery biopsy was performed and consistent with a diagnosis of giant cell arteritis. Treatment with high-dose oral steroids was continued.
