Peters Anomaly

An infant was referred into Bascom Palmer for congenital corneal opacity in the right eye. Exam in clinic is notable for corneal opacity with what appears to be lens material in the anterior chamber and calcific changes of the lens. Exam under anesthesia revealed lens adherent to the posterior cornea and a diagnosis of type II Peters anomaly was made. To prevent deprivational amblyopia, the lens material was surgically removed. Penetrating keratoplasty nor optical iridectomy were deemed necessary in this case. Posterior exam revealed retinal pigmentary changes. Genetic testing revealed a pathogenic variant in NR2E3, associated with enhanced S-cone syndrome. To our knowledge, this is the first case of possible enhanced S-cone syndrome co-occurring with Peters anomaly.
