Traumatic Optic Neuropathy

A patient presented to a tertiary trauma center after a fall from standing. On arrival to the hospital, he was found to have extensive facial fractures and was describing loss of vision in the left eye. Detailed review of the CT scan revealed a bone fragment protruding into the optic canal, and a diagnosis of traumatic optic neuropathy was made. The patient was administered high dose corticosteroids, but did not experience improvement in the vision. Treatment options at this time are limited and prognosis for TON is notoriously poor, but a novel multimodal approach has shown promise in animal models.
