Traumatic Cataract from Glaucoma Drainage Implant Tube Contact

A patient with a history of pediatric glaucoma and buphthalmos in the setting of Bardet-Bield syndrome presented to the emergency room with acute progressive vision loss in the left eye after a seizure. The patient prior to injury had no light perception in his right eye from glaucoma and had a baseline visual acuity of 20/150 in the left eye. In the emergency room, his left eye had a vision of hand motion and intraocular pressure of 10 mmHg . Slit lamp examination demonstrated a white cataract in the left eye with a long inferonasal tube in the anterior chamber that was in contact with the lens. A Bscan ultrasound was performed which demonstrated no acute posterior pathology in the left eye. The patient underwent complex cataract surgery of the left eye with shortening of the inferonasal tube. He returned to his baseline vision after post-operative week 1.
