West Nile Virus Chorioretinitis
A patient with a recent history of meningoencephalitis presents with blurry vision and increased number of floaters noted after discharge. During hospitalization, he required IV treatment with Meropenem and Acyclovir due to unclear etiology. Past ocular history was consistent with mild non-proliferative diabetic retinopathy in both eyes and a small branch retinal vein occlusion in the left eye. Visual acuity was 20/20 in the right eye and 20/25 in the left eye. Intraocular pressure was 12 and 14. Slit lamp exam of the anterior segment was within normal limits. The posterior segment exam disclosed scattered punctate, linear chorioretinal scars typical of West Nile chorioretinitis. Fundus autofluorescence disclosed multiple lesions with central hypofluorescence and peripheral hyperfluorescence consistent with inactive lesions. Serological studies for West Nile were positive. Due to the inactivity of the lesions, no further treatment was required.
