A child presented to the ER with blurry vision and eye pain. Exam disclosed unilateral optic nerve edema and decreased color vision; subsequent MRI disclosed optic neuritis. Lab testing demonstrated +Aqp4 antibodies, and the patient subsequently developed longitudinally extensive transverse myelitis. The patient was treated with IV steroids and transitioned to immunomodulatory therapy (IMT) with rituximab which stabilized her newly diagnosed Neuromyelitis Optic Spectrum disorder (NMOSD). Upon cessation of IMT, she developed recurrent ocular symptoms and was thought to have flare-ups of her NMO. She was treated with repeat steroid therapy and recommended to restart IMT.
