A patient who underwent an intravitreal anti-vascular endothelial growth factor injection developed a white cataract after his procedure. Careful examination and ultrasonography revealed a white cataract with a posterior capsule violation, likely from the intravitreal injection. The patient underwent a cataract surgery. This case will highlight important pre-operative considerations when planning for surgery and intra-operative techniques on how to manage cataracts when there is a pre-existing posterior capsule violation.
