Elevated Episcleral Venous Pressure

A 59-year-old male presented with blurred vision in both eyes. He was referred to the glaucoma service for evaluation of elevated intraocular pressure. On examination, the patient was observed to have blood in Schlemm’s canal and bilateral intraretinal and subretinal fluid. Of note, the patient had a known history of severe pulmonary hypertension. The patient was subsequently diagnosed with elevated episcleral venous pressure with secondary open angle glaucoma and secondary central serous retinopathy. The mainstay of treatment for this patient included aggressive management of pulmonary hypertension and use of topical and oral glaucoma medications. Surgical management can be difficult in such cases.
