Peters Anomaly Type II

A 2-day-old male was referred by a neonatologist for evaluation and management of corneal clouding and buphthalmos of the left eye. The patient was found to have unilateral elevated intraocular pressure, corneal clouding, enlarged corneal diameter, and a poor red reflex. B-scan ultrasound in clinic revealed anterior segment abnormalities, increased axial length of the eye, and marked optic disc cupping. The patient underwent exam under anesthesia, which demonstrated findings consistent with Peters’ anomaly type II and secondary glaucoma. Vitrectomy and lensectomy were performed at the time of exam under anesthesia. The patient is being monitored closely to determine the next steps of management. Future considerations include genetic counseling and testing, management of glaucoma (medically and possibly surgically), management of corneal clouding, and visual rehabilitation.
