Abstract
A patient with a past medical history significant for biopsy-proven ocular cicatricial pemphigoid (OCP) was referred for cataract evaluation in the right eye. At presentation, her vision in the right eye was hand motion, and she had been off systemic
immunosuppression for 9 months. She was re-started on mycophenolate mofetil and achieved disease quiescence within three months. Keys to surgical success during her cataract surgery included ocular surface exposure, intra-ocular visualization with
a light pipe, and judicious use of conjunctival manipulations and cautery. OCP does not preclude successful cataract surgery, and proceeding with cataract surgery after systemic disease control can greatly improve quality of life for patients.
