Abstract
A patient with 7 years of blurry vision in the right eye presents to clinic for evaluation. He reports no recent acute changes in vision and no precipitating events in the past. He noticed the blurry vision incidentally while driving when he covered his left eye. He has no known past ocular history and his medical history is only significant for hypertension, for which he takes lisinopril. His visual acuity in the right eye was 20/150+2. The visual acuity in the left eye was 20/20. Slit lamp examination was normal in both eyes. Posteriorly, the fundus examination in the right eye was notable for a large melanocytic lesion covering the optic nerve with brown debris emanating into the vitreous. There was also brown debris collecting in the inferior aspect of the vitreous cavity. A diagnostic pars plana vitrectomy was planned. Pathology revealed benign melanocytic cells without evidence of malignancy. A diagnosis of melanocytoma with vitreous seeding was made.
