Abstract
This is a case of an elderly woman with pathologic myopia referred for asymptomatic worsening macular schisis in the right eye on optical coherence tomography (OCT). Fundus examination showed pathologic myopia and a posterior pole staphyloma. She was diagnosed with myopic tractional maculopathy (MTM). Over the course of 3 years she developed foveal detachment, outer and inner lamellar holes, complete resolution of her maculopathy, recurrence of schisis and foveal detachment and then again complete resolution of the maculopathy. Fluorescein angiography and OCTA did not show evidence of choroidal neovascularization. Decision was made to observe the right eye at every visit during this course. In fact, 5 years after her initial visit, her maculopathy remained resolved without intervention. This case supports the fact that MTM is a dynamic progressive disease that is an interplay of different forces exerted by the vitreous and expanding globe.
