Abstract
This is a case of a patient with 3 years of blurry vision. He was diagnosed with polypoidal choroidal vasculopathy by an outside physician and was receiving monthly aflibercept injections with documented worsening of vision and appearance on OCT. ICG was repeated on presentation to our institution with confirmation of choroidal polypoid lesions. The decision was made to treat the patient with photodynamic therapy and continue monthly aflibercept injections. This presentation covers the demographics, differential diagnosis, pathophysiology, major studies in the literature that guide the care of these patients, and alternative treatment modalities that have met with varying success. After supplementing with PDT, vision steadily improved in the right eye and has remained stable in the left eye for this patient. Intraretinal and subretinal fluid has improved in both eyes. This case provides valuable teaching points for the treatment of refractory PCV.
