Topic outline

  • Grand Rounds

A patient with a PMHx of HTN presented to BPEI with redness and discharge of the left eye for 3 months. Review of systems was negative for any systemic symptoms. Visual acuity of the left eye was 20/80, IOP was 13 mmHg, there was no APD, and EOMs were full. Examination of the left eye revealed eyelid margin thickening, perilimbal conjunctival thickening with injection, an inferotemporal symblepharon, and temporal fornix shortening. The patient underwent a conjunctival biopsy that was positive for mucous membrane pemphigoid/ocular cicatricial pemphigoid (MMP/OCP). The patient was started on methotrexate and will be followed up in one month. MMP/OCP is a rare cause cicatricial conjunctivitis and often requires combinations of systemic immunosuppression depending on severity in order to achieve disease remission.
