    An elderly patient presented to the glaucoma department for follow up examination of her primary open angle glaucoma. Her IOP was above goal and progression was noted on Humphrey visual fields of both eyes. She underwent uncomplicated cataract extraction with intraocular lens implant combined with trabeculectomy to lower her IOP. Her IOP was controlled post-operatively with one laser suture lysis and the bleb was noted to be diffusely elevated. At post-operative week seven, a bleb leak was noted 2mm posterior to the limbus. A bandage contact lens was placed, which did not stop the bleb leak. The patient underwent a bleb revision for a persistent bleb leak. On post-operative day one, the bleb was Seidel negative with an elevated IOP, therefore, a second laser suture lysis was performed. At post-operative week three, another bleb leak was noted. A bandage contact lens was placed in hopes of the leak closing without surgical intervention. The surgical technique and post-operative course is documented in this presentation.
