Abstract
A patient presented to the Bascom Palmer Emergency Room with 2 months of worsening blurry vision in the right eye greater than left. On exam, the patient was found to have a choroidal mass in the right eye consistent with a choroidal melanoma and had a concurrent exudative retinal detachment. At the same time, the patient had a large advanced basal cell carcinoma on the glabella, left medial canthus and with orbital extension. Diagnosis of the ocular melanoma was confirmed with BScan and fine needle aspiration biopsy. Given the patients poor vision in both eyes, globe sparing management was pursued for both pathologies. The patient underwent plaque brachytherapy, endolaser, and intravitreal avastin with good regression of the tumor. A recalcitrant exudative retinal detachment was drained externally with good effect. The patient was also started on vesmodegib for basal cell carcinoma and had a good response. Ultimately the patient had excellent visual outcomes and the globe was spared bilaterally.
