Abstract
A patient from Haiti presented to the eye emergency room with redness and blurry vision of the right eye for 10 days. The patient was noted to have acute anterior uveitis with normal dilated fundus exam. The patient was started on topical prednisolone drops with some improvement. Upon tapering the topical steroid, they developed recurrent eye redness and blurry vision. At this time, the patient was noted to have multiple choroidal granulomas and a peripapillary lesion as well. The patient’s lab workup was notable for positive Quantiferon assay. Subsequent chest radiograph was negative for pulmonary tuberculosis. The patient was treated with a total of 3 months of 4-drug therapy (Rifampin, isoniazid, pyrazinamide, and ethambutol), followed by four months of two-drug therapy with rifampin and isoniazid, with resolution of choroidal granulomas, and a plan to continue treatment until completion. This patient demonstrates a case of tuberculoid choroiditis which responded well to antituberculous therapy.
