Morgagnian Cataract

A patient presented to the Bascom Palmer Eye Institute with progressive painless vision loss. Vision was light perception. He was found to have a morgagnian cataract, confirmed on ultrasound biomicroscopy. The planned surgical approach was manual small incision cataract surgery. During capsulorrhexis creation, a distinct separate capsular flat was noted posterior to a complete curvilinear capsulorrhexis. This capsular flap was also extended 360 degrees. The dense nucleus was then prolapsed and removed through the scleral tunnel. A 3-piece intraocular lens was placed in the sulcus with optic capture. On post-operative slit lamp examination, the posterior capsule was intact and separate from the two distinct anterior capsules. This “double-ring” sign is a rare but reported finding of unknown etiology but may represent a spectrum of disease of true lens exfoliation. To our knowledge, this is the first case reported of a double ring sign in the setting of a morgagnian cataract.
