82-year-old female presented to BPEI from the Cayman Islands with 3 weeks of floaters and decreased vision in the right eye. Visual acuity was hand motion and posterior exam revealed vitreous hemorrhage, submacular hemorrhage, and a retinal arterial macroaneurysm (RAM) superotemporally in the right eye. As patient had significant visual impairment due to foveal involvement and had already failed anti-VEGF injection, she underwent a pars plana vitrectomy + subretinal tPA + air fluid exchange. Patient regained vision and at post-operative 1 year her visual acuity was 20/25. Her course and the treatment of RAMs are discussed in this presentation.
