Grand Rounds
A patient with a past ocular history of HSV keratitis and chalazia s/p biopsy presented with severe light sensitivity and intermittent ocular redness and discomfort in OS with symptoms present for ~ 10-15 years. The slit lamp exam was notable for an elevated, gelatinous lesion located in a paracentral/inferior location, extending into the mid-stroma in the left eye. Examination of the right eye was unremarkable. The patient was started on prednisolone drops in OS and valacyclovir. Shen then underwent a superficial keratectomy for lesion diagnosis, and biopsy results showed amyloid deposition. The patient was lost to follow-up, discontinued systemic medications, and returned with the initial symptoms, worsening visual acuity and regrowth of the corneal lesion in OS. The patient was biopsied, and the results showed amyloid deposition. Post-operatively, the vision improved in the left eye, with no signs of a lesion recurrence.
