Grand Rounds
An infant with history of preterm birth presented to the Bascom Palmer emergency room with a rapidly progressive left upper lid mass. Evaluation showed non-axial proptosis and a large superonasal, vascular mass deep in the left orbital rim. MRI brain and orbits with and without contrast showed a soft tissue mass between the left globe and medial orbital wall enhancing on post gadolinium. Left orbitotomy with biopsy was done, and pathology showed vascular proliferation with lobules of small to medium sized capillaries separated by fibrous septa that stained positive for GLUT-1, CD31 and CD34 and stained negative for D2-40. This confirmed the diagnosis of infantile hemangioma. The patient was treated with oral propranolol and had improvement in mass size and proptosis.
