A patient presented with 1 day of decreased vision in 2016. Exam disclosed OS>>OD optic nerve edema with visual field defects without other findings. The patient was treated with steroids without much improvement and lost to follow up. The patient then over the following years re-presented multiple times with bilateral sequential optic nerve edema eventually with macular edema. Lab workup during this time and neuroimaging were all largely within normal limits without clear etiology. The patient was diagnosed with recurrent idiopathic neuroretinitis. The patient was ultimately started on immunosuppression with azathioprine and has since remained in remission. She ultimately maintained good central vision with permanent peripheral visual field defects. Recurrent Idiopathic Neuroretinitis is a difficult to manage condition without clear treatment and with limited evidence for prevention with immunosuppressive therapy.
