    A 79-year-old male with history of Fuchs endothelial dystrophy of both eyes s/p penetrating keratoplasty of both eyes (1980s), repeat penetrating keratoplasty of left eye (2018) and regmatogenous retinal detachment of left eye s/p repair with silicon oil who presents with culture positive endophthalmitis of the left eye which began following fresh water exposure on a hike in the Dominican Republic 5 months following repeat penetrating keratoplasty and 5 months prior to presentation. The patient underwent repeat penetrating keratoplasty with open-sky explantation of IOL and lens capsule complex. His intraoperative corneal, lens capsule, and aqueous sample cultures all grew achromobacter xylosoxidans. His post-operative course was uncomplicated and at post-operative month 2 he had no evidence of recurrent infection, his graft was functioning well, his BCVA had improved to 20/150 and his retina exam remained stable.
