Grand Rounds
A patient presented to the emergency department with 2 days of floaters and pain with ocular movements. He endorsed exposure to stray cats and being scratched by a kitten. Best-corrected visual acuity was 20/20 in both eyes. Anterior segment exam was unremarkable and posterior exam revealed optic nerve edema in the left eye and multifocal retinitis in both eyes. Optical coherence tomography showed hyperreflectivity of the inner retina in areas of retinal lesions. FA showed optic disc leakage in the left eye with centripetal staining of the retinal lesion. Work up revealed positive IgM and IgG for Bartonella henselae and patient was subsequently diagnosed with bilateral ocular bartonellosis. He was treated with doxycycline, Rifampin and oral steroids.
