Abstract
A patient with a history of Best Disease mutation in BEST1 presented with a sudden onset of blurriness and distortion in her left eye (OS) for three weeks. The patient's family history reveals genetic and visual implications, with her mother being affected and her sister carrying the mutation without manifestation. Her visual acuity was 20/20 in the right eye, 20/40 OS. On presentation, she had macular thickening with subretinal deposits and fluid. Additionally, she was 25 weeks pregnant. Optical Coherence Angiography revealed type 1 macular neovascularization. She was treated with an intravitreal dexamethasone implant with improvement. Other treatment options include observation and cautious use of steroids, laser photocoagulation, anti-vascular endothelial growth factor (VEGF), and photodynamic therapy. This is a unique presentation in a patient with unilateral Macular Neovascularization secondary to best disease in pregnancy.
