    A myopic patient with daily orthokeratology lens use presented for eye pain and decrease in vision of both eyes for one week. Patient had no changes in his normal use of lenses. On examination, patient demonstrated findings concerning for an infectious keratitis. Cultures were initially negative and confocal microscopy was performed for further elucidation. While not definitive, the constellation of findings and confocal imaging raised concern for an acanthamoeba infection. Later, cultures of one eye grew a gram negative bacillus. While believed to be contaminant, appropriate coverage was still provided to include this micro-organism. Patient was treated with appropriate acanthamoeba coverage as well and is currently being followed closely in cornea clinic. Orthokeratology lens use was discontinued.
