    A patient presented with a pigmented conjunctival lesion in the right eye of insidious onset and progressive course. She was found to have a ciliochoroidal melanoma with extraocular extension and underwent excision of the extraocular extension, I-125 plaque brachytherapy, and tutoplast graft over the area of extrascleral extension. She later developed scleral melting that was treated by another scleral patch graft. Several weeks later, the patient developed acute loss of vision in both eyes and had signs of panuveitis with choroidal thickening and leakage on angiography. She was diagnosed with sympathetic ophthalmia and treated with systemic steroids and methotrexate that lead to improvement of the inflammation and vision.
