Grand Rounds
A patient with a history of a central retinal artery occlusion presented with acute vision loss in his right eye and was found to have a branch retinal artery occlusion. A fluorescein angiogram showed retinal vasculitis of both eyes and the patient was diagnosed with Susac Syndrome. He was transferred for stroke workup and extensive workup was unremarkable. The patient was started on IV solumedrol, IVIG and mycophenolate. While tapering of immunotherapy, the patient was noted to have recurrent vasculitis and was started on azathioprine and increased IVIG and steroids. On follow up, his imaging showed resolution of segmental arterial wall hyperfluorescence. We will discuss the clinical features of Susac syndrome and multimodal imaging findings, as well as the management and treatment.
