Abstract
A patient who is 6 weeks post-partum from a C-section due to prolonged labor and hypertension presents to the emergency department for right eye pain and swelling for 1 month, but worse over the last 2 weeks. She also has associated right-sided headache. Examination revealed 20/20 vision with full extra-ocular movements, corkscrew-like conjunctival vessels, tortious retinal veins. An MRI of the brain and orbits with and without contrast, as well as an MRA and MRV of the head were performed. There was evidence of a carotid-cavernous sinus fistula on imaging due to a dilated superior ophthalmic vein. The patient was then evaluated by neurosurgery and diagnosed with a Type D carotid cavernous sinus fistula and subsequently underwent transvenous coiling and is doing well post-operatively.
