Abstract
A patient presented with acute vision loss of the left eye following a blow to the left side of the head and left eye. On examination, the visual acuity of the left eye was hand motions. He was noted to have splitting of the iris stroma in the affected eye, with similar but less severe iris irregularities in the asymptomatic fellow eye. He was also diagnosed with a rhegmatogenous retinal detachment with a giant retinal tear of the left eye. On anterior segment OCT and Ultrasound biomicroscopy, there was clear separation between the anterior and posterior iris stroma in both eyes, with a preserved iris pigment epithelium, pathognomonic for iridoschisis. The patient denied any past ocular history. There was no history of syphilis. There was no evidence of corneal decompensation or glaucoma. The patient underwent left retinal detachment repair. He required no intervention for bilateral iridoschisis.
