Neuromyelitis Optica Spectrum Disorder

A patient with a past medical history significant for autism spectrum disorder and amblyopia of the left eye presented with one day of acute decrease in visual acuity of the left eye. The initial exam revealed inferior optic nerve edema of the left eye, and consequential MRI of the brain and orbits with and without contrast demonstrated optic neuritis of the left eye. The patient was started on a course of five days of intravenous steroid therapy without adequate response. Further diagnostic workup revealed positive antibodies against Aquaporin-4, leading to a diagnosis of Neuromyelitis Optica Spectrum Disorder. The patient was treated with five additional days of intravenous steroids and sent to a neurologist for further management. The patient was started on rituximab for maintenance therapy. The patient was then monitored thereafter with imaging to observe for development of new lesions. Ultimately, with appropriate and timely diagnosis and management, the patient returned to baseline visual acuity.
